2024 Impact 100 Indy Grant Application

Use this form to upload the PDFs you have created for your 2024 Impact 100 Greater Indianapolis Grant Application. If you upload the wrong PDF, you can click the upload button and load the correct one. Use Save and Continue at the bottom of the page often as you work; you will be sent a link to go back to the form to finish.

Required Grant Application PDF Attachments

The following steps ask for attachments. It is recommended that you use Save and Continue often. Insider’s tip: After you click Save and Continue, you can use the back arrow to go back to your form where you left off.
Please upload the PDF of your Grant Application. You must name the attached file as follows: 2024Grant-[FAC]-[NFP], where [FAC] is the short name for the Focus Area Committee you chose to review your LOI and [NFP] is the name of the nonprofit submitting the LOI (if you use a shortened version of the NFP name, please use the same shortened version to name all submitted files). Your Grant Application may not exceed six (6) pages.

You must upload a copy of your organization’s Executive Director pf CEO in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-exec-resume.pdf following the naming conventions for [FAC] and [NFP].

You must upload a copy of the resume and/or job description of the proposed project leader in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-leader-resume.pdf following the naming conventions for [FAC] and [NFP].

You must upload a copy of the board of directors list (including corporate or other affiliation of each member, and note the officers) in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-board.pdf following the naming conventions for [FAC] and [NFP].

You must upload a copy of the organization’s budget for the current fiscal year, indicating income and expenses, in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-budget.pdf following the naming conventions for [FAC] and [NFP].

You must upload a copy of the year-to-date organizational financial information for the current fiscal year in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-current-financial.pdf following the naming conventions for [FAC] and [NFP].

You must upload a copy of last fiscal year’s financial information in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-previous-financial.pdf following the naming conventions for [FAC] and [NFP].

You must upload one of the following in PDF format: CPA Audit (for organizations with annual budget over $1million); Financial review by an accounting firm (for organizations with budget less than $1 million); OR An explanation for your organization not having an Audit or a Review. NOTE: If submitting an Audit or Review, please provide two years of financial history in your audit/review. If your current audit/review does not contain two years of history, please submit the previous audit/review as well. Please name the most recent review file as follows: 2024Grant-[FAC]-[NFP]-audit-review.pdf, and if you add a second file, name it as follows: 2024Grant-[FAC]-[NFP]-audit-review2.pdf

    You must upload a document verifying your 501c(3) status in PDF format. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-501.pdf. DO NOT UPLOAD EINs, 990s, or other documentation in this field. If your organization is using a fiscal sponsor and does not have their own 501c(3), please attach a PDF of the fiscal sponsorship agreement as documentation of the relationship. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-Sponsor.pdf.

    If your organization is using a fiscal sponsor and you uploaded a PDF of the fiscal sponsorship agreement in the previous step, please attach a PDF of the IRS determination letter for that fiscal sponsor. Please name the file as follows: 2024Grant-[FAC]-[NFP]-SponsorIRS.pdf. DO NOT UPLOAD any other documentation in this field.

    This is optional, but you can upload a PDF copy of your most recent strategic plan here, or a PDF document with a link to an online version. Please name the attached file as follows: 2024Grant-[FAC]-[NFP]-strategic-plan.pdf following the naming conventions for [FAC] and [NFP].

    Submission

    Don’t click Submit until you are sure you are finished with your Grant Application submission. Once you click Submit, your submission is final. You will receive an email confirming it was sent and a second email with a copy of your submission.
